HyCoSy
Fertility investigation
A HyCoSy is a fertility investigation used to look at the uterus and fallopian tubes in more detail. It is carried out as part of a transvaginal ultrasound scan, using contrast fluid to help show the uterine cavity and whether the fallopian tubes appear open.
For many patients, a HyCoSy scan is an important part of fertility testing before treatment is planned. It can give our doctors helpful information about anything that may be affecting the uterus or fallopian tubes and, in turn, what options may be most suitable.
The HyCoSy procedure is minimally invasive and usually only takes a little longer than a standard transvaginal ultrasound. It should not be painful, although some people may notice mild discomfort or light cramping. Our team will guide you through the scan carefully and go at a pace that feels manageable for you.

What is a HyCoSy?
HyCoSy stands for hysterosalpingo-contrast-sonography. It is a type of HyCoSy ultrasound used to assess the uterine cavity and fallopian tubes. It is very similar to a transvaginal ultrasound, but with an extra step: a small amount of contrast fluid is gently introduced to make the images appear more clearly.
This allows our doctors to assess whether the fallopian tubes appear open and whether the uterine cavity looks suitable for pregnancy or fertility treatment. A HyCoSy test can sometimes show information that would not be visible on a routine ultrasound alone, which is why it is often recommended as part of a wider fertility investigation.
In some cases, an AMH test may also be recommended alongside a HyCoSy scan so your results can be reviewed as part of your wider fertility picture.

Why do we perform the HyCoSy?
We use a HyCoSy test because the uterus and fallopian tubes play an important role in conception and pregnancy. If the fallopian tubes are blocked or the uterine cavity is affected in some way, this can influence which fertility options are most appropriate.
A HyCoSy procedure gives our team useful information before treatment begins. It can help us understand whether treatment, such as IUI, may be appropriate, or whether another option may give you a better chance of success. If your results suggest that the fallopian tubes may be affecting your fertility, our doctors may discuss treatment options, such as IVF, with you.
The results are always reviewed alongside your wider fertility picture, including your medical history, symptoms, previous results, blood tests and ultrasound findings. If your results suggest that the fallopian tubes may be affecting your fertility, our team will explain what this means clearly and support you through your next steps.

A HyCoSy may be useful if you have suspected or diagnosed endometriosis, as it can help assess the uterus and whether the fallopian tubes appear open. However, it does not diagnose all forms of endometriosis. Your results will be reviewed alongside your symptoms, history and any other relevant tests.
Our team will advise you on sex after HyCoSy based on your individual situation and how you feel after the test. Some patients may prefer to wait until any spotting or cramping has settled. If you have been given specific aftercare guidance, follow that advice.
The difference between HSG and HyCoSy is mainly how the tubes are assessed. HSG usually uses X-ray imaging and dye, while HyCoSy uses ultrasound and contrast fluid. Our team can explain which test is most appropriate based on your circumstances.
A HyCoSy is a diagnostic test, so its main purpose is to provide information. Some studies have suggested that tubal flushing may be associated with improved conception rates in some cases, but this should not be considered guaranteed.
